Software Summary

The Health Analytics Reporting Platform (HARP) is built and operated by HealthInfoNet for its Health Information Exchange (HIE) participants and other authorized organizations. Leveraging a robust clinical data set, HARP contains both aggregate and patient-level analyses of individuals receiving care throughout the state of Maine.

Risk Profile Dashboard

  • Added the Risk Class filter option

Patient Risk Dashboard

  • Added the Risk Model filter option

  • Added the Risk Class filter option

Use case: The addition of the above filter options to their respective reporting dashboards allows users to better identify and retrieve information on specific patient risk populations.

Scenario: Find patients at higher risk for developing Type 2 Diabetes in the next 12 months.

  • On the Risk Profile dashboard, apply the following filters options:

    • Risk Model = Type 2 Diabetes

    • Risk Class = High, Very High

    • 90D Risk Change = Increase

  • Select the Patient Risk dashboard

    • Note: The Risk Class and 90D Risk Change filter selections from the Risk Profile dashboard will propagate; no additional action needed

  • Re-apply the filter option, Risk Model = Type 2 Diabetes to narrow the table columns to just the risk model of interest

  • Retrieve a list of all attributed patients that have increased in risk over the last 90 days to either a High or Very High risk class for Type 2 Diabetes.
