Demographics
Component Overview
In the upper-left hand corner of the "Patient Summary" view is the "Demographics" component. This component provides additional demographic information about the selected patient.
Field | Description |
|---|---|
Full Name | The patient’s most recently reported first and last names. Displayed as “last name, first name.” |
Sex | The patient’s most recently reported legal/insurance sex value. Valid values include Male, Female, X, and Unknown. |
Date of Birth | The patient’s date of birth. |
Race | The patient’s standardized race category based on their most recently reported race value. Valid values include AIAN (American Indian or Alaska Native), Asian, Black (Black or African American), Multi (two or more races), NHPI (Native Hawaiian or Pacific Islander), White, and Unknown. |
Ethnicity | The patient’s standardized ethnicity category based on their most recently reported ethnicity value. Valid values include Hispanic, Non-Hispanic and Unknown. |
Address | The patient’s most recently reported primary street address, city/town, state, and ZIP code of residence. |
Phone | The patient’s most recently reported telephone number. |
Primary Care Provider | The patient’s most recently reported primary care provider. |
Emergency Contact | The patient’s provided emergency contact information. |
