Attribution by Patient (H)

Overview

The Attribution by Patient (H) utility provides detailed information about the specific entity(ies) to which patients are attributed within reporting for each active Attribution Type value(s). This view provides a horizontal display (H) of patient attribution information, whereby each patient is given a single record in the table per Attribution Type value with separate columns for each of the Attribution Level value(s) to which they belong within reporting. Description

Attribution Type

The name of the specific patient panel(s) that are currently available within reporting, inclusive of all available active Attribution Type values.

HIN ID

The HealthInfoNet-assigned identifier that identifies a unique patient within the HIE’s data warehouse.

Patient ID

The participant-supplied identifier(s) that identifies an attributed patient based on where they received care.

An individual patient may have many participant-supplied identifiers if they have been treated at multiple care locations over time. Valid values are inclusive of Medical Record Number (MRN) values and other source system identifiers submitted via participants' Admission, Discharge, and Transfer (ADT) HL7 feeds, as well as Medicaid ID values (sometimes referred to as 'A' numbers) shared by the Office of MaineCare Services to uniquely identify its Medicaid members.

Note: Regardless of how many Patient IDs that an individual patient may have, they will only have a single HIN ID assigned.

The first and/or last names of an attributed patient.

HIN ID

The HealthInfoNet-assigned identifier that identifies the attributed patient within the HIE’s data warehouse.

Note: This field can be used to join patient records across all patient-level table exports within reporting.

Patient Name

The first and last names of the attributed patient.

Date of Birth

The date of birth of the attributed patient.

Attribution Type

The name of the specific patient panel(s) to which the attributed patient belongs within reporting.

Note: Users can select the + icon found when hovering over the Attribution Type field name in the table to reveal an additional Patient ID field. This field provides the participant-supplied identifier(s) that identifies the attributed patient in the patient panel defined by the Attribution Type value.

For example, even though a unique patient may be included in multiple participants' attribution files with separate Patient ID values coinciding with each participant’s respective source systems, this field will only show the patient’s Patient ID value used in the selected Attribution Type value.

Attributed Provider

The name of the provider (e.g., MD, DO, NP, PA, etc.) to which the patient is attributed.

Note: At this time, patients can only be attributed to providers via Participant-Supplied Custom Attribution.

Attributed Site

The name of the location(s) and/or department(s) to which the patient is attributed.

Attributed Facility

The name of the healthcare facility(ies) to which the patient is attributed.

Attributed Group

The name of the medical group or broader healthcare institution(s) to which the patient is attributed.

Attributed System

The name of the health system or integrated delivery network to which the patient is attributed.

Attributed Program

The name of the value-based program(s), such as an Accountable Care Organization to which the patient is attributed.
