Attribution by Entity
Overview
The Attribution by Entity utility provides aggregate counts of attributed patients per entity within reporting for each active Attribution Type value(s). For more information, visit the Patient Attribution methodology section.
Click the settings icon 
located in the upper-left corner of the utility to learn more about its available features and functionality.
Filters
Filter | Description |

Attribution Type | The name of the specific patient panel(s) that are currently available within reporting, inclusive of all available active Attribution Type values. For more information about available Attribution Type values, visit the Patient Attribution methodology section. |

Count of Attributed Patients by Attribution Entity per Attribution Type | Attributed Program | The name of the value-based program(s), such as an Accountable Care Organization to which patients are attributed. For more information about HealthInfoNet’s hierarchical relationships used for attribution purposes, visit the Attribution Level & Entity methodology section. |
Attributed System | The name of the health system or integrated delivery network to which patients are attributed. For more information about HealthInfoNet’s hierarchical relationships used for attribution purposes, visit the Attribution Level & Entity methodology section. | |
Attributed Group | The name of the medical group or broader healthcare institution(s) to which patients are attributed. For more information about HealthInfoNet’s hierarchical relationships used for attribution purposes, visit the Attribution Level & Entity methodology section. | |
Attributed Facility | The name of the healthcare facility(ies) to which patients are attributed. For more information about HealthInfoNet’s hierarchical relationships used for attribution purposes, visit the Attribution Level & Entity methodology section. | |
Attributed Site | The name of the location(s) and/or department(s) to which patients are attributed. For more information about HealthInfoNet’s hierarchical relationships used for attribution purposes, visit the Attribution Level & Entity methodology section. | |
Attributed Provider | The name of the provider (e.g., MD, DO, NP, PA, etc.) to which patients are attributed. Note: At this time, patients can only be attributed to providers via Participant-Supplied Custom Attribution. Pts Attributed | The count of unique patients attributed to the identified entity, per Attribution Type. |
