Demographic Search
Function Overview
Users can select the "Demographics Search" option from the "Patients" function to search for patients for whom they would like to view health records. Users can enter the appropriate combination of demographic information for the desired patient and initiate the search to generate possible matches.
Required demographic field combinations include:
First Name + Last Name (RECOMMENDED FOR OPTIMAL MATCHING)
Last Name + Date of Birth
Facility + Medical Record Number (MRN)
When results from users' demographic searches return, there may be more than one patient that meets the provided criteria. To confirm that the correct patient is selected, users can review the provided demographic details associated with each resulting record.
Field | Description |

Privacy Seal (i.e., Lock Icon) | Indication with an opened lock icon of whether the patient’s health record has been opened by the user within the last three (3) days. For more information, visit the section on Patient Record Access Request. |
Last Name | The patient’s most recently reported last name. |
First Name | The patient’s most recently reported first name. |
Middle Initial | The patient’s most recently reported middle name initial. |
Date of Birth | The patient’s date of birth. |
Sex | The patient’s most recently reported legal/insurance sex value. Valid values include Male, Female, X, and Unknown. |
City | The patient’s most recently reported primary city/town of residence. |
State | The patient’s most recently reported primary state of residence. |
ZIP Code | The patient’s most recently reported primary ZIP Code of residence. |
Opt-Out Date | The date when the patient opted-out of sharing their health information with HealthInfoNet, if applicable. For more information, visit the section on Consent Choices. |
Mental Health Consent | The date when the patient opted-in to sharing their mental health information with HealthInfoNet, if applicable. For more information, visit the section on Consent Choices. |
HIV Consent | The date when the patient opted-in to sharing their HIV information with HealthInfoNet, if applicable. For more information, visit the section on Consent Choices. |
