HARP New User Request Form

Overview

HealthInfoNet’s analytics participants may request HARP access for individuals serving their organization depending on staff and workflow needs. The following section describes the process by which analytics participants can choose to share new user request(s) with HealthInfoNet.

Process Requirements

In order for analytics participants' Account Authorizers to request new user accounts for their organization, they must follow these steps:

  1. Confirm and/or request the latest HARP New User Request Form with HealthInfoNet’s Customer Care team (customercare@hinfonet.org).

  2. Populate the form with the desired new user attributes, per the Field Requirements section below.

  3. Save the form:

    1. as an Excel .xlsx file

    2. with a file name of “[organization_description] [today’s date: mmddyyyy]” (e.g., “Acme Facility 03062023”)

  4. Share the populated form via email with HealthInfoNet’s Customer Care team (customercare@hinfonet.org).

The following table outlines the fields included in the HARP New User Request Form template, and each field’s requirements.

The date when the Account Authorizer requested the new user’s HARP account.

The first name of the requested new user.

user_lname

The last name of the requested new user.

user_email_address

The email address of the requested new user.

user_job_title

The job title of the requested new user.

user_organization_id

The HealthInfoNet-defined organization identifier that identifies a unique organization to which the new user should be associated for reporting purposes.

Valid values are specific to each organization; see the User Organization reporting dashboard for a full listing of all relevant identifiers to which users can be associated at your organization.

user_organization_description

Using the User Organization reporting dashboard, match the selected user_organization_id with the HealthInfoNet-defined organization description to help HealthInfoNet’s Customer Care team to confirm that the selected organization identifier aligns with the intended organization.

user_clinical_portal_id

The existing Clinical Portal User ID of the requested new user, if known. If not known, leave null (i.e., blank).
