Patient Condition/Event Flags

On the Insights dashboard of each HARP module, where available, Patient Condition/Event filtering identifies patients with a relevant diagnosis in their medical history to allow for enhanced population-level filtering. These flags are identified in several ways.

The following Patient Condition/Event flags are identified within HealthInfoNet's Predictive Risk Modeling for patients who have a diagnosis of the condition/event (i.e. with a risk class of “Present” and a risk score of 100):

  • Alzheimer’s Disease

  • Cardiac Dysrhythmias

  • Chronic Kidney Disease

  • Chronic Obstructive Pulmonary Disease

  • Congestive Heart Failure

  • End-Stage Renal Disease

  • Hypertension

  • Lung Cancer

  • Type 2 Diabetes

Additionally, the following Patient Condition/Event flags were developed in partnership with Maine CDC. Specifications for these can be found below.

Coronary Artery Disease (CAD) Condition Prevalence Flag

Definition: A positive flag identifies a patient who has had a relevant diagnosis code at any time in the last 3 years.

Inclusion: Inpatient, Emergency Department, or Outpatient encounters with a relevant ICD-10 diagnosis code anywhere in the record and an admission or encounter date within the last 3 years.

Exclusions:

  • Patients who are deceased on or before the run date.

  • I25.110 Atherosclerotic heart disease of native coronary artery with unstable angina pectoris

  • I25.111 …… with documented spasm

  • I25.112 Atherosclerotic heart disease of native coronary artery with refractory angina pectoris

History of COVID-19 Related Hospitalization Prevalence Flag

Definition: A positive flag identifies a patient who has had a relevant diagnosis code at any time since March 1, 2020.

Inclusion: Inpatient encounters with a relevant the primary or secondary ICD-10 diagnosis code.

Exclusions:

  • Patients who are deceased on or before the run date.

Myocardial Infarction (MI) Condition Prevalence Flag

Definition: A positive flag identifies a patient who has had a relevant diagnosis code at any time in the last 3 years. This flag includes both Acute Myocardial Infarction and Older/Health Myocardial Infarction.

Inclusion: Inpatient, Emergency Department, or Outpatient encounters with a relevant ICD-10 diagnosis code anywhere in the record and an admission or encounter date within the last 3 years.

Exclusions:

  • Patients who are deceased on or before the run date.

Peripheral Artery Disease (PAD) Condition Prevalence Flag

Definition: A positive flag identifies a patient who has had a relevant diagnosis code at any time in the last 3 years.

Inclusion: Inpatient, Emergency Department, or Outpatient encounters with a relevant ICD-10 diagnosis code anywhere in the record and an admission or encounter date within the last 3 years.

Exclusions:

  • Patients who are deceased on or before the run date.

Transient Ischemic Attack (TIA) Condition Prevalence Flag

Definition: A positive flag identifies a patient who has had a relevant diagnosis code at any time in the last 3 years.

Inclusion: Inpatient, Emergency Department, or Outpatient encounters with a relevant ICD-10 diagnosis code anywhere in the record and an admission or encounter date within the last 3 years.

Exclusions:

  • Patients who are deceased on or before the run date.

Lastly, a Patient Condition/Event Flag of “No Chronic Condition” indicates patients who do not meet the criteria for any of the above.
