Communication Package
Overview
HealthInfoNet’s Communication Package has been carefully designed to help inform and educate participating organizations’ staff and their patients about HealthInfoNet and the state of Maine's statewide Health Information Exchange (HIE). The Communication Package includes materials that provide a high-level overview of HealthInfoNet and its suite of services, articulate the value proposition and purpose of the HIE for providers and patients alike, and share most recent patient consent forms, staff talking points, and legal references for connected organizations to use in their internal intake procedures.

HealthInfoNet General Medical Information Opt-Out Form 
Allows patients to opt-out of sharing their general medical information with HealthInfoNet

Allows patients to share their general medical information with HealthInfoNet after previously opting-out
HealthInfoNet Sensitive Information Opt-In Form
Allows patients to opt-in to sharing their sensitive information with HealthInfoNet
HealthInfoNet Sensitive Information Opt-Out Form
Allows patients to opt-out of sharing their sensitive information with HealthInfoNet
Per Maine State law (22 MRSA 1711-C), the first time that a patient visits an HIE-participating organization, the patient must be notified that the organization participates in the HIE and offered a HealthInfoNet General Medical Information Opt-Out Form that gives users the option to opt-out of sharing their general medical information with HealthInfoNet and the statewide HIE.
HealthInfoNet’s consent forms (print versions only) are now available in French, Somali, and Spanish languages for ease of use. Visit our website to find and begin using these translated copies.
